1. Agreement to Terms
By accessing our website or using RevalonMD LLC's services, you agree to be bound by these Terms and Conditions. If you do not agree, please do not use our services.
2. Services
RevalonMD LLC provides medical billing, coding, credentialing, and revenue cycle management services to healthcare providers. The specific services, fees, and terms for each client relationship are set forth in a separate Service Agreement.
3. Payment Terms
Clients are invoiced monthly for services rendered during the prior month. Payment is due within 10 days of invoice receipt. RevalonMD LLC accepts payment via bank ACH transfer, wire transfer, or credit card. Late payments may be subject to a 1.5% monthly finance charge.
4. Credentialing Services
Credentialing fees are due upon engagement. If a payer panel is closed and an application cannot be processed, the fee will be adjusted or applied to an alternative payer enrollment at the client's direction.
5. Termination
Either party may terminate the service agreement with 30 days written notice. Upon termination, RevalonMD LLC will provide all client records and data in a standard format. Outstanding invoices remain due upon termination.
6. Confidentiality & HIPAA
RevalonMD LLC agrees to maintain the confidentiality of all client and patient information in accordance with HIPAA and applicable state laws. A Business Associate Agreement is executed with every client prior to the commencement of services.
7. Limitation of Liability
RevalonMD LLC's liability is limited to the fees paid by the client during the preceding three months. RevalonMD LLC is not liable for payer decisions, claim denials resulting from clinical documentation issues, or delays caused by payers or regulatory bodies.
8. Governing Law
These Terms are governed by the laws of the State of New York. Any disputes shall be resolved through binding arbitration in New York County, New York.
